(March 2007) Either entitys name with address can be used in the signature line. If the restaurant name/address is used, it needs to be preceded with the words “Manufactured for” or “Distributed by” or similar wording that expresses the facts. If the manufacturer’s name/address is used, no lead-in is necessary. For reference, this question is covered in the Code of Federal Regulations (21CFR101.5).
