I just got my new license back. I had requested a name change. Why wasnt my name changed on my license?
In order to change your name on your teaching license, we need some sort of legal proof of the name change. A photocopy of the appropriate document (marriage certificate, divorce decree, etc.) will be sufficient. If you provide this documentation within thirty days of the printing of your license, we will change the name and print a new license for you at no charge. Beyond that date, you can submit verification of name change, and we will be happy to make the change that will appear on future licenses; there is a charge only if you ask for an updated copy of the license.
