I just drove by a pasture and saw a horse that was so thin his ribs were showing. Do horses need hay and other supplements besides the grass thats in the pasture?

Unless the pasture is in very good condition, most horses need supplements of some kind, like hay or grain. And pasture isn’t always of the same quality all year long. The new grass in the spring doesn’t have the value that grass later has, nor are all grasses created the same. Other factors could be the horse’s age and teeth, or even if it has worms.
