I installed Reference Manager and see the menu options in the Tools menu of Microsoft Word, but I don’t see the toolbar. Where is it?

It’s most likely an installation/configuration problem. Start by re-installing on a fresh copy of Eclipse and a new workspace. If you’re installing under Eclipse 3.4, you are probably running into a p2 bug. See the Eclipse 3.4 cache problem workaround.
