a. Upon review and receipt of all required information by the Provider Enrollment department, a paper welcome letter will be mailed within 5 business days notifying you of your DOL Provider Number. The welcome packets are forwarded to the correspondence address as indicated on the provider enrollment application. If no other address is indicated, then the welcome packet will be forwarded to the providers physical address.
