I have secondary prescription drug coverage through Providence Health Plans. How do I get reimbursed for copays?
• If your primary and secondary coverage are through Providence Health Plan, your prescriptions will automatically receive coordination of benefits at the pharmacy. If your pharmacy needs assistance, they may call the pharmacy department at 503-574-7400 or 877-216-3644. • If your primary coverage is through another insurance carrier, fill out a prescription drug reimbursement request form (PDF), attach your itemized pharmacy receipt, and mail both to the address listed on the form. Keep a copy of both the completed form and your receipt for your records. We are working toward establishing a feature to allow secondary coverage to automatically receive coordination of benefit at the pharmacy.
