I have my assessments set to automatically submit at the time limit, yet some students show as having submitted late. Why?

When the time limit is reached, the student sees a dialog box that informs them the time limit has been reached and that they must submit. The actual submit time is recorded when they click the acknowledge button in that dialog box. The are not allowed to continue or change the assessment once the time limit has been reached.
