You will receive your Medicaid card on the first of each month. Fairfield County Job and Family Services is NOT able to issue any replacement cards, written verification or billing numbers to customers who are on a Managed Care Plan. . If you are covered through a Managed Care Plan (Anthem, CareSource or Molina), you will need to contact your plan to request a new card.
