A charge can be filed by any current or recently terminated employee, against any private employer, employing less than fifteen (15) employees during 20 or more calendar weeks within the calendar year of or preceding the alleged violation; including employment agencies and labor organizations; any lending institution. The employer must be located within the corporate limits of the City of Peoria. If you believe that you have been discriminated against in the area of Employment and/or including sexual harassment on the job, you need to fill out the WORKPLACE COMPLAINT INTAKE FORM equal-opportunity-forms and return the completed form to the EOO via mail, fax or email.
