The Jewish General Hospitals address is 3755 Cote Ste-Catherine Road. It is sided by three streets: Cote des Neiges, Cote Ste-Catherine and Legare streets. Parking is off Legare Street. Before the time of your appointment with Dr. Mark Miller (Infectious Diseases Department, Room G-140), you will need to make a JGH Hospital Card. You can make a hospital card in room E-0014 (this pavilion is the Legare street entrance [across from the parking lot]). Once you have your card, please come to room G-140 to register for your appointment with Dr. Miller. If you have a vaccination booklet (carnet), please bring it with you.
