Mail order prescriptions include receipts and/or packing slips that list the number of prescription refills that have been shipped to you. When you submit program certificates for the multiple prescription refills you received through the mail, enclose the receipt or packing slip and also include 1 program certificate for each prescription refill (2 trays, 60 vials) of RESTASIS® Ophthalmic Emulsion.
