If you do not have a Claimant ID number but know that you purchased or leased a vehicle from Reliable Chevrolet, Reliable Automotive, Reliable Imports and RV, or Van Chevrolet-Cadillac during the class period, you may print the Notice and Proof of Claim from this website, complete and mail it to the Reliable Settlement Claims Administrator at PO Box 2061, Faribault, MN 55021-2061. If you are unable to print the Proof of Claim, you may also request that it be mailed to you by sending an email to: mail@ReliableSettlement.com. Please be sure to complete the Approximate Date of Purchase or Lease, Make and Model of Automobile, and Amount of Processing Fee Paid (if known) portions of the form so we can properly research your eligibility for each claim. Your Proof(s) of Claim must be postmarked on or before March 17, 2011.
