I clicked on a form title to open it, but the Terms of Use policy keeps appearing. How do I open the form I want?
Each new session (that is each time you visit the website) on our website requires that you agree to the Terms of Use policy before you are allowed to download forms. This notifies you of your responsibilities as a user of this website. When you reach the Terms of Use page, you will find two buttons at the bottom of the page. Electing to choose the “Agree and Download” button once is sufficient for each active session, and it will not appear again during the session.
