I changed my name, dosent match my birth certificate.. I need a passport?
Now this will depend on the state you reside in and what all needs to be changed.Now if you legally changed you name(court ordered)you should have taken that court issued info to your vital statistics dept and they will never change a birth record they will amend it(meaning nicely cross out old info and add new info and affix the county seal, your past will never be erased)anyone that needs your birth record will know who you once were.Now if by some change your sex needs to be changed as well,then different rules apply.Like the surgeon licensed in the US must supply certain post op paperwork of what they have done too you as well as another doc appointed by the court to do a post op exam as well to verify sex change,then a Name and Sex form must be submitted and approved before any amendments can be made.So yes “Stacey will still be legally named John if the state does not approve form.Now once the birth record is fixed everything else will be easy(ssn card,license.id card etc.
I had the same situation. I had to have a notarized letter explaining the situation. My elementary grades, my high school grades, my college transcripts from all three of my degrees, newspaper clippings from stories with both my old and new name, witness statements, my mothers driver’s license and birth certificate, credit reports, and about every other thing you could think of. THey sent it all into the whatever agency is in charge of that at a federal level and to my surprise I got my passport in about 6 days. The more stuff you give them the easier it will be, they said that is why I got mine back so quick because i provided everything they needed the first time.
