When one spouse/civil union partner dies and both names appear on the personalized, preprinted Homestead Rebate Application Packet, the eligible widow(er)/surviving civil union partner must file a paper application and enclose a copy of his or her social security card and a copy of the deceased partners death certificate to have the rebate check issued in the survivors name only. If the application is filed online or by phone, both names will appear on the rebate check.
