It is necessary for a service member to first request their transfer through their chain of command. After doing so, if the service member would like Congressman Crenshaw to make an inquiry, please send the Privacy Act form, which you can complete and print in Step Two, and any supporting documentation to the Jacksonville office of Congressman Crenshaw.
