I am on methadone maintenance and would like to switch to buprenorphine maintenance. Is it possible to switch directly from methadone to buprenorphine?

That depends on your methadone dose. Because buprenorphine can cause a person to go into withdrawal if their opioid tolerance is too high, most methadone patients will need to have their dose lowered to approximately 30mg/day before being switched to buprenorphine.
