Application forms are available online at www.cxc.org . The forms must be completed by the teacher, signed by the Principal and sent to CXC Headquarters at the earliest opportunity. Forms may also be obtained from the CXC Headquarters in Barbados or its Offices in Jamaica. Persons who have previously applied, need not reapply, but should notify the Council if there has been a change in any of the information previously supplied.
