For state non fingerprint based information for other than the subject of record: you must submit a form provided by the department. The form requires the subject of record to authorize release of the criminal history information to a 3rd party. The subject and the requester must sign the form and the record subject must sign an unsworn falsification statement. The form can be mailed or hand delivered to the department. For fingerprint based state background checks the following information must be provided (mailed or delivered): • A full ten-print applicant fingerprint card • A letter of explanation as to why they need this information • A current mailing address • A Money order in the amount of $35 payable to the state of Alaska.
