Previous Northwestern students returning to start a new half-time or full-time academic program must complete and submit a new Admission Health Record (AHR). On the Admission Health Record it asks if you were ever a student at Northwestern, mark “Yes” and list the dates of attendance. Review the immunization requirements to make sure that you are up to date for vaccinations based on your new entrance date. Once we receive your Admission Health Record, we will combine it with your old file and use the immunization records you previously submitted. If you need to update a vaccination we will inform you of that in our confirmation email. If a vaccination is needed, you will be expected to complete it by the appropriate deadline, or you will risk incurring a penalty.
