You should provide us with a clearly worded request for the information you require. You should include if possible the student’s full name, date of birth, title and dates of course(s) undertaken. We can confirm the year a degree was awarded and the degree title. To obtain any more information than this, you will have to include with your request a copy of an official release form signed by the student. You can fax us this information to +44 (0)20 7955 6099, email it to registry@lse.ac.uk or post it to the Student Services Centre. Please provide us with your fax number, email or postal address and instructions on how you want our reply to be returned to you. I am applying for a Graduate programme and I must provide evidence that no disciplinary action was taken against me by LSE. The registry team will complete the form for you. Please email it to registry@lse.ac.
