If you have received your medical ID Card, please submit your claim along with a completed “Medical Benefits Request Form” to the address printed on your card. Be sure to include your Member ID number and the employee’s Social Security number (so we can identify your account). Also, please be sure that your payment to the provider is clearly indicated on the bill. Medical Benefit Request forms can be obtained from your employer’s benefits office, or by calling your Member Services office at the toll-free number listed on your ID card. If you do not have an ID card yet, please contact your employer’s benefits office to obtain this toll-free number. If you have not received your Medical ID card, please contact your employer’s benefits office to obtain the mailing address to which to submit your claim form.
