If employees need to request reimbursement for their flu shot, the employee should contact Keith Ramalho from Maxim Healthcare Services at 410-910-1376 or keramalh@maxhealth.com and provide him with their insurance information, including their SHP member ID number, two-digit dependent number and group number, as well as the date and location of the flu shot clinic the employee attended.
