How would we be able to connect DCFS clients with GAIN Services?
You may complete an Information Request Form (IRF) for your client and return it to your Linkages GAIN Services Worker (LGSW) so that the clients current status with DPSS can be reviewed. If GAIN eligible, they may be given an expedited appointment to participate in the program or to add the GAIN service component/ancillary/transportation that they wish to include in their already existing GAIN case. Please note that your LGSW may also be included/invited to a TDM held for the family and in which case an IRF is not needed because services will be offered or expedited as part of the TDM Action Plan.
