A multifaceted approach to generate public awareness has been developed for MFP. Transition Coordination agencies will be encouraged to advise facilities, families, etc. of the agency’s enrollment as a Medicaid provider agency for Transition Service/funding. Transition Coordinators/Case Managers may visit the local nursing facilities, intermediate care facilities and long-stay hospitals to work with the administration in identifying persons eligible for successful enrollment in the MFP program. Each identified participant will have the right to choose if they want to transition to the community.
