When an employee has advance knowledge of the need for FMLA-protected leave, he notifies his supervisor. The supervisor notifies the human resources office, and that staff determines if the employee meets FMLA eligibility requirements (having worked for the state for 1250 hours during the previous 12 months and having worked for the state for at least 12 months). If the employee has FMLA leave available from the annual 12-week allowance, the human resources office notifies VSDP. VSDP then contacts the employee and sends forms for doctor certification of the illness or injury. The employee is responsible for follow-up.
