How will the vendors obtain the survey forms?
CMS will disseminate the survey forms via the MA & PDP Web site in electronic format in both English and Spanish for the 2011 data collection, i.e. 2010 reference year. The surveys will be made available through the web site in early summer 2010 so that plans and survey vendors can see what basic information will be asked. However, the final survey forms for 2011 will not be available until December 2010.
