During the application process, states will evaluate their progress and state and submit strategic and operational plans. Based on the assessment, each applicant will indicate where they are in the process: no existing plan; non-compliant existing plan; compliant existing plan. This Strategic Plan will be a tool for ONC and the state and/or state designated entities (SDEs) to monitor, communicate and track progress throughout the four-year performance period. ONC will work closely with each recipient to identify where they stand along a continuum of planning and implementation. Additionally, ONC will provide ongoing program direction to assist states and SDEs in addressing the stages of HIE, as well as the five domains to enhance the effectiveness and relevance of the state HIE initiatives to local and national health improvement goals.
