In late 2003 the NPSA will roll out a national reporting and learning system to collect and analyse information on patient safety incidents from local NHS organisations, NHS staff, patients and carers. The NPSA will analyse patient safety incidents to understand the root causes behind things that go wrong and identify the patient safety issues that need to be addressed. Where risks are identified, work will be undertaken on solutions to prevent harm, specifying national goals and establishing mechanisms to track progress. 8.
