Additional support from hospice is especially important when the type of care necessary increases to the general inpatient care level. Currently, Rainbow Hospice Care’s in-home patients (including those residing in assisted living facilities and nursing homes) needing general inpatient care are transferred to a hospital setting—usually at either Fort Memorial Hospital (Fort HealthCare) or UW Health Partners Watertown Regional Medical Center. Rainbow Hospice team members work together with hospital staff to provide necessary coverage. While hospital RNs may administer medications and respond to other clinical needs, the hospice team is responsible for managing and administering the overall plan of care (including medical, emotional and spiritual care) for the patient and his or her family.
