Some changes may occur secondary to the HCPCS Level II codes because they are occasionally not as specific as the local “99” codes. Most frequency limitations were established through contracts with the manufacturers that were completed in 2008. Most of the changes to frequency limitations were already published in the appropriate provider manuals for each HCPCS Level II code.
