Patients must qualify before being seen. You must come in and provide proof of residency, proof of one month of household income, and fill out the State required paperwork prior to being scheduled the next available provider visit. The hours to complete this paperwork are Monday, Tuesday, or Thursday between the hours of 10 am 2 pm.
