How will Sindecuse Health Center determine priority of vaccination of students if health histories are not available for students?
Should the quantity of vaccination be severely limited, the burden of proof of health history will rest with students and parents. Students may sign a Release of Information Form to allow Sindecuse Health Center professionals to obtain past medical history through fax or verbal communication with community health care providers.
