Each of the four categories of work (Performance Management, Policy and Workforce Development, Public Health System Development/Redevelopment, and Best Practice Implementation) has a sub-goal and measurable indicator upon which applicants will build their individual performance plans. (See Section II. Program Implementation section of the FOA for details.) Applicants will submit proposals that include what actions they will take to contribute to achievement of the category sub-goal and how they will measure and report their individual progress in terms of the associated measurable indicator, including baselines for the specific activities they will undertake. Go to “Section V. Application Content,” and “Part D – Performance Plan” of the FOA for more guidance on the required elements of the proposal performance plan.
