How will EDCD Transition Coordination interface with service facilitation?
There is no change. Interfacing with the Service Facilitator is no different than with any other provider. If the person elects to receive consumer directed services under a waiver, the Transition Coordinator or case manager will include that in the service plan, and provide the participant or his/her representative a list of Service Facilitators.
