How will Delta Dental of MN handle claims for orthodontia treatments already in progress?
• The orthodontist must submit a copy of the original claim, full treatment fee, estimated treatment length (not month remaining, but original treatment length), original date of placement, previous carrier information, coordination of benefit information and current signature of the orthodontist. The orthodontist must submit this information to Delta Dental of MN even though the documentation may have already been submitted to your previous dental carrier. Delta Dental of MN is not able to process a claim until the required information is received from the provider. Delta Dental of MN will review the treatment plan and determine its liability in the absence of other coverage. In the event there is other coverage Delta Dental of MN will then coordinate benefits. Delta Dental of MN will pay no more than its contracted maximum lifetime amount for orthodontic services minus the previous carrier payments.
