On enrollment, every member covered under packages will be issued a `Members’ Manual’ which would bear a photograph of the individual, basic personal and medical details, eligibility limits, terms and conditions of our package, the list of our network of providers, contact details, emergency contact numbers etc. This `Members’ Manual’ shall be produced at the time of availing the healthcare facilities at any of the Network of Providers. The provider shall on verification of the details contained in the said Manual shall extend the facilities on credit. On discharge of the patient, the provider shall make entries in the Manual details of the facilities extended, amounts utilised by the member etc.
