There was originally swearing and sexual connotation. Most of the Dragon Quest games originally had a “bunny girl” who supposedly gave you a *puff-puff*. I have heard that this was a BJ, but others say it has to do with a facial message using a comfortable part of the female anatomy which I tend to feel has more validity.
