How was the data collected for CheckPoint?
The medical and surgical data are collected from individual patient charts and submitted to a third party data system by the hospital staff. The error prevention data are collected from patient charts and entered directly into the CheckPoint system by the hospital staff. These data collection process reflects actual care given and error prevention practices in Wisconsin hospitals.
