Rational Function Tester (RFT) is a functional testing tool and Rational Test Manager (RTM) is a test management tool. First install both the tools then it requires integration part and this integration some times done automatically. Open logtypes in RFT if you identify TestManager is one of the log type then these two tools are integrated successfully. In RTM tester can plan and add the test cases after adding test cases tester can assign a script to testcase. If you have any issue plz revert back.
