How to sign up for the Head Start Program?
Please complete and return this application quickly. (Or call us, and we will come and get it or help you with it!) We need for you to include copies to verify: all your income since January 1, 2008 (example: pay stubs, W-2, income tax return, child support, disability, unemployment, Social Security/SSI, TANF/OWF, self employment income, and any other income); your child’s immunization record; Foster Care Status, if any; and birth certificate (can also use hospital certificate, crib card, Medicaid card, etc.). If you send an original, we will copy it and return it to you.. Although income eligibility is based on 130% of the Federal Poverty Guidelines, a few may be over this income. All applications are accepted and considered. Age, income, disability of the child, and family situations are used to prioritize applications, so let us know if any of these things change.
