This can be obtained by applying on the prescribed form to the Controller of Examinations, University of Kerala, Thiruvananthapuram-34 with a fee of Rs.200/-, an affidavit from the Notary Public on Rs.25/- stamp paper, stating that the certificate had been lost irrecoverably and the course certificate from the Principal of the institution where the candidate had studied for that degree course.
