How to create an evaluation online?
• Go to http://eval.umn.edu • Click on the “Set up an evaluation or review results” link in the upper left corner • Click “Create an Evaluation” link • Click on “Create an early or end-of-semester semester evaluation”, depending on what type of evaluation you are trying to set up • Select appropriate form to use • Click “Use this Template” button You will need to submit a separate evaluation request for each course you wish to have evaluated.
