Requirements for NEW Businesses: Application Form (New) Barangay Clearance Engineering Inspection Certificate Health Inspection Certificate Fire inspection Certificate Waste Management Seminar Certificate Occupational Permit Photos of establishment (showing signboard & sidewalk) DTI / SEC Registration Community Tax Certificate Others Procedure: Applications must be received and approved by the Business Permits and Licensing Office (BPLO) Attach all required supporting documents then submit the same to BPLO DTS reference slip will be given to the applicant The business establishment will be inspected When everything is in order, assessment will be prepared and to be paid to the Treasury Office teller immediately.
