How to apply Degree Certificate (In place of lost Original Certificate)?
• Duplicate Certificate (In place of lost Original Certificate) Application Form is available at S.K.U. Press, Sri Krishnadevaraya University. • Application can be Downloaded from www.Sri Krishnadevaraya.ac.in or www.ouresults.com. Enclosures: • Affidavit on Non Judiciary Stamp Paper of Rs. 20/- sworn before Notary • Xcopies of memorandum of marks, lost certificates and other relevant documents. • 3 Passport size Photographs. • D.D. in favour of ‘Controller of Examinations, SKU, Anantapur. • D.D. from any Nationalized Bank’s only. • D.D. valid upto one month from the date of its issue. • At the time of receiving Certificate any Photo Identity Card should be produced. Fee Details : • Rs. 1000/- Note: • These instructions are applicable for all courses (except External Courses) • Certificate (In place of lost Original Certificate) will be issued after 30 days. • Counter receipt valid upto 90 days only.
