In all areas outside of NYC the reports are sent by regular mail to the address noted on the provider form. The mailing address is: Division of Epidemiology, P.O. Box 2073, ESP Station, Albany, NY 12220-0073. The hard copy form that is sent to the DOH does not contain any reference to HIV or AIDS. Therefore, if the form was inadvertently sent to the wrong address, the receiver would not know that the information contained on the form was HIV-related.
