How should the electronic DD Form 1380 (field medical cards) get to the combat support hospital (CSH)?
Theater Medical Data Store (TMDS) is the best method for providers at different MTFs to view a patient’s electronic health record (EHR). A patient’s EHR is collected and entered into the MC4 handheld device and then synched to Composite Health Care System II-Theater [CHCS II-T (AHLTA-T)] or entered directly into CHCS II-T (AHLTA-T). Within hours after an encounter is signed, providers and commanders can view the patient data via TMDS anywhere in the world. This allows providers at higher level of care to see the care previously given on the battlefield.
