Please remind parents to report all immunizations to the Immunization Program at Ottawa Public Health (OPH) one of the following ways: • Telephone: 613-580-6744 ext. 24108 • Fax: 613-580-9660 • E-mail • Online Reminding parents to report their immunizations may reduce any future need for OPH to contact your office regarding immunization records. For more information, please contact OPH and ask to speak with staff from the Vaccine Preventable Diseases Branch.
