How should a Medicare Supplement claim be submitted?
Send us a copy of the Medicare Explanation of Benefits form received from Medicare (“EOMB”) with patient’s full name and policy number clearly written on it. Most people choose to write it on top, where there isn’t a lot of Medicare preprinted information. (Make a copy for yourself if you like). That’s all we need to process the claim!
