2A. Premiums are due every six months. The payroll report for the first 6 months of the year is sent to the employer in July. The payroll report and premium payment are due August 31. The payroll report for the second 6 months of the year is sent to the employer in January of the following year. This payroll report and premium payment are due February 28 (or 29).
